Lofan - we regret that the stickers did not apply properly on your pet's crate. We have tested this product extensively inhouse on our crates with no problems and are not sure why they did not adhere. We have added a recommendation that the crate be cleaned with rubbing alcohol prior to the application of the stickers. Airline Veterinary Certificate
The form is NOT airline specific rather country specific in terms of validity time. Some countries require different vaccinations and chips to be implanted prior to rabies vaccination. Do your homework before throwing money for stuff your vet already has. If your vet doesn't have the form then you should look for a vet that is USDA certified here: https://vsapps.aphis.usda.gov/vsps/public/VetSearch.do.

Hello Bob - we regret the misunderstanding with regard to your purchase. You are correct that the Airline Health Certificate is not airline specific, nor is it to be used as a substitute for a veterinary health certifciate to import a pet to another country internationally. This form is for commercial airlines who require a health certificate for pets flying in the cabin, as accompanied checked baggage or air cargo. Health certificates for international travel as well as very detailed instructions on importing a pet to a specific country can be found in our Pet Passport section. https://www.pettravelstore.com/categories/Pet-Passports/ The USDA does not publish a general airline health certificate for commercial airlines that we are aware of.
