I’m not sure what the purpose of this meeting was about. Did I just pay $160 for a form to have my primary physician to fill out?? My “doctor” wasn’t able to answer any of my questions and the “appointment” lasted less than two minutes.

Form was too blurry after being forwarded to the state twice and now it's been over 30 days so I will have to pay an in person dr to make sure my forms go through waste of my money.
