It would be helpful if the doctors would fill out the medical conditions on the OMMA application. Mine were not. If it isn’t, that will require a third submission and fee.

Hi Shawn, as noted on the OMMA form, the medical conditions are optional and left blank for privacy. It will not cause any issues with your application (we help hundreds of patients every day and it has never been an issue). Thanks
